Active confluence: A proposal to integrate social and health support with technological tools

Ageing demographic structures produce higher rates of physical and cognitive impairments, lack of independence, and social isolation of old people. This poses challenges to general and mental health care. In this chapter, we present a framework for seamlessly integrated, web-based digital systems to enhance active ageing and independent living by unobtrusively monitoring older people's health signs and providing feedback both to them and to important nodes in their social support network.
