Contributo farmacêutico na reconciliação da terapêutica, a experiência ibérica: uma revisão sistemática

Abstract

Patient transition between primary and hospital care may potentiate medication discrepancies. Medication reconciliation programs are one of the preventive strategies. The present study aims to evaluate how Pharmacists, due to their expertise and presence in the medication circuit, can play an important role in medication reconciliation in Portugal and Spain. A systematic review of the literature was performed, searching MEDLINE, Elsevier and RCAAP databases for articles published between 2007 and 2012. The studies were then independently analysed by two reviewers. Of the 54 articles identified, only 11 met the inclusion criteria. Seven papers present a type of prospective study, two are retrospective and one is a descriptive study. In all studies, discrepancies in medication were identified. Pharmacist intervention in therapeutic reconciliation showed a significant reduction of the discrepancies, especially of the unintended. Few hospitals in Portugal have this process implemented, although their implementation allows economic gains and benefits the welfare of patients. In general, the studies included in this review lead to the conclusion that Medication Reconciliation is an indispensable activity to improve patient safety, aiming at reducing medication discrepancies in all care interfaces, especially in emergency medical services.
